Dr. Auzins graduated from Indiana University Dental School in 1996 and completed his Oral & Maxillofacial Surgery Residency in 2000. He became Board Certified in 2002, and renewed his certificate in 2012. He served the United States Navy until 2003 with two tours to the Middle East. He was stationed on the aircraft carrier, USS Theodore Roosevelt and had a faculty position at Portsmouth Naval Hospital.
He has been in private practice since 2003 and teaches at OHSU part–time. At the state level, he currently sits on the Multnomah Dental Society, the Free to Smile Foundation, and the Oregon Society of Oral and Maxillofacial Surgery. He is also a member of the anesthesia committee of the Oregon Board of Dentistry.
Dr. Auzins patient care extends beyond his practice in Portland to those around the world. Through the Free to Smile Foundation, he is helping patients in developing countries by providing cleft lip and palate surgery. As part of an all volunteer team, he has treated children in China, Tibet, Guatemala, and Niger. Locally Dr. Auzins volunteers for Medical Teams International, providing oral surgery in their dental van for people in need. He and his team routinely participate in the Mission of Mercy (MOM) 2010, 2011, 2013, 2016, Dr. Auzins and his family enjoy spending time in the great outdoors. He enjoys downhill skiing, cycling, hiking, and is also an amateur jazz drummer.
Carlos M. Ugalde, DDS, MS
Dr. Ugalde grew up in Costa Rica and earned his Doctor of Dental Surgery degree from the Latin American University of Science and Technology in 2003. He then enrolled at the Ohio State University and completed his Master of Science degree and a four-year residency in oral and maxillofacial surgery. He subsequently completed a fellowship in cleft and craniofacial surgery at the University of Oklahoma.
Dr. Ugalde has been in private practice in Washington since 2010 and served as an affiliated instructor in the Oral and Maxillofacial Surgery Department at the University of Washington School of Dentistry from 2011–2014. Dr. Ugalde currently holds an affiliate Assistant Professor position at Oregon Health Sciences University. He has authored and co-authored multiple scientific articles in peer-reviewed journals and lectured nationally and internationally about cleft lip and palate, orthognathic surgery, head and neck infections, facial lacerations, and facial trauma. He enjoys volunteering with the Mission of Mercy here in Portland and has also participated in surgery missions to Mexico and Guatemala to treat under-privileged children with cleft lips and palates.
Dr. Ugalde and his family recently relocated from Washington to Oregon. Outside of dentistry, he spends his free time with his wife and three kids. He also enjoys mountain biking, aviation, playing the piano, Formula One racing, soccer, and tennis. He is fluent in English and Spanish.
Thomas A. Kolodge, DDS, MD
Dr. Kolodge is a board certified oral and maxillofacial surgeon. He was raised in Beaverton, Oregon and graduated from Aloha High School. He attended George Fox University on a soccer scholarship and earned his degree in Biology. He lived in New York City, while attending NYU School of Dentistry. Dr. Kolodge went on to complete medical school and residency in oral & maxillofacial surgery at the University of Nebraska Medical Center.Dr. Kolodge first decided to become an oral surgeon in 1998, after observing Dr. Johnson while an undergraduate student at George Fox. A special friendship was formed and the two discussed dreams of becoming partners one day after Dr. Kolodge completed his years of training. Following his graduation from dental school, medical school and residency – he joined Dr. Johnson and formed Mountain View Oral Surgery.In his spare time, Dr. Kolodge enjoys adventuring with his wife, Sarah, and their son. Anywhere from the mountains to the beach, they love to spend their days outside. When not exploring, he loves to golf and sing for his beloved Portland Timbers with the rest of the Timbers Army.
Brett A. Ueeck, DMD, MD, FACS

Dr. Ueeck is a board certified oral and maxillofacial surgeon with a wide breadth of clinical experience. He was born and raised in Oregon’s Willamette Valley and moved to Seattle for his undergraduate studies. Shortly after graduation, he married his wife Melissa and they enjoyed traveling throughout Asia and Europe. He enrolled in OHSU and earned both dental and medical degrees and completed residency in oral and maxillofacial surgery. After residency, Dr. Ueeck joined the faculty at OHSU where his primary focus was complex surgical cases and the education of the oral and maxillofacial surgery residents. He remains an affiliated clinical professor at OHSU.
A chief area of surgical interest to Dr. Ueeck is cleft lip and palate. He worked at Shriners Hospital for Children on the cleft lip and palate team for many years and continues to travel internationally to help children in third world countries and developing nations affected with cleft and other craniofacial conditions. Locally, Dr. Ueeck volunteers with Medical Teams International, Compassion Connect, and Mission of Mercy.
Beyond clinical practice, Dr. Ueeck also lectures frequently on various topics such as orthognathic surgery, cleft lip and palate, head and neck cancer, medical emergencies, and dental implants. He contributes to the surgical literature by authoring articles and textbook chapters, as well as reviewing articles for publication in several of the surgical journals. He maintains several professional memberships in his surgical specialty, including the American College of Surgeons.
Dr. Ueeck holds several board positions and is the current chief of oral and maxillofacial surgery at St. Vincent hospital. He serves as the state chairman for the committee on anesthesia and is responsible for the oversight of the office anesthesia safety evaluation program.
Outside his professional responsibilities Dr. Ueeck enjoys spending time with family and friends, mountaineering, skiing, and fly-fishing.
Russell Lieblick, DMD

Dr. Lieblick, a native Oregonian, completed his undergraduate degree at the University of Oregon in 1995. He graduated from the University of Louisville School of Dentistry in 2000 and completed his surgical residency at MetroHealth Medical Center in Cleveland, Ohio.
Licensed in Oregon and Washington, Dr. Lieblick is on the medical staff of Mount Hood Medical Center as well as the downtown Legacy Hospitals, Providence Hood River Memorial Hospital, Providence Milwaukie Hospital, and Mid-Columbia Medical Center. Dr. Lieblick is board certified by the American Board of Oral and Maxillofacial Surgeons and is president of Oregon Society of Oral and Maxillofacial Surgeons. He is a Member of the American Association of Oral and Maxillofacial Surgeons, the Western Society of Oral and Maxillofacial Surgeons, the Oregon Dental Association, the American Dental Association and the Multnomah Dental Society. He is a Fellow of the American Society of Dental Anesthesiologists.
When away from the office, he is busy playing with his two children and enjoying activities with his wife, Nina. They enjoy visiting friends and family from all over the country and like to expose their children to as many new activities as possible.
Daniel Petrisor, DMD, MD

Dr. Petrisor is a board-certified surgeon with training in microvascular reconstructive surgery, head and neck surgery and oral and maxillofacial surgery. He holds a dental degree from Oregon Health & Science University and a medical degree from the University of Texas Southwestern Medical School. In addition to medical school, he also completed his residency at the University of Texas Southwestern Medical Center and Parkland Memorial Hospital in Dallas, TX. Following residency, Dr. Petrisor completed a fellowship in head and neck surgery and microvascular reconstructive surgery at Louisiana State University Health Sciences Center in Shreveport. He then completed an additional fellowship in microvascular reconstructive surgery at the University of Florida-Jacksonville.
Dr. Petrisor has a special interest in treating patients with benign and malignant tumors of the head and neck. He also has training in reconstruction of complex deformities of both hard and soft tissues. This combination of advanced training allows for a patient’s own tissues to be used for reconstructive surgery at the same time as their cancer surgery. Additionally, he is part of a team of physicians, dentists, nurses, nutritionists, therapists and social workers all working together to provide each patient the latest in comprehensive treatments for a variety of cancers of the mouth, head and neck.
Joel K. Hopkin, DMD, MD

Dr. Joel Hopkin is a graduate of Oregon Health & Sciences University School of Dentistry. He received his bachelors degree in the life sciences from the University of Idaho.
He completed his medical degree at Texas Tech University Health Sciences Center. He then completed a general surgery internship at Baylor University Medical Center in Dallas, Texas. Dr. Hopkin completed his Oral & Maxillofacial Surgery Training at Baylor University Medical Center/Baylor College of Dentistry in Dallas, Texas.
Dr. Hopkin is a Board Certified Oral & Maxillofacial Surgeon. Dr. Hopkin has published several scientific articles and has lectured at national meetings as well as to the dental community. He is a diplomate of the American Board of Oral & Maxillofacial Surgery.
He is a clinical assistant professor in the Oral and Maxillofacial Surgery Department at Oregon Health and Sciences University in Portland, Oregon. He is a member of the Oregon Dental Association, Oregon Medical Association, American Dental Association, Oregon Society of Oral and Maxillofacial Surgeons, Western Society of Oral and Maxillofacial Surgeons, and American Association of Oral and Maxillofacial Surgeons. He is on staff at Salem Health, Samaritan Albany General Hospital and Northbank Surgical Center.
Dr. Hopkin met his wife Cristina while attending medical school. They have two boys and one girl. His interests include fishing, skiing, outdoor and family activities.
David W. Howerton, DMD
Dr. David Howerton was raised in Portland, Oregon and attended Portland State University for his undergraduate studies. He graduated from Oregon Health & Sciences University School of Dentistry.
He then completed a one year general practice residency at Loma Linda Medical Center. After this he returned to Oregon and completed his Oral & Maxillofacial training at Oregon Health & Sciences University.

Dr. Howerton has published numerous scientific articles and is active in lecturing for the dental community. He became a diplomate of the American Board of Oral & Maxillofacial Surgery in 1991.

He is a member of the American Dental Association, American Association of Oral & Maxillofacial Surgeons, Western Society of Oral and Maxillofacial Surgeons, and the Oregon Society of Oral & Maxillofacial Surgeons. Dr. Howerton is the proud father of three boys including twins. His interests include basketball, traveling, snow skiing, fly fishing, kayaking, biking, and family activities.
Gabriel M. Kennedy, DMD

Dr. Gabriel Kennedy was raised in eastern Oregon and attended Oregon State University for his undergraduate studies. He graduated from the University of Connecticut School of Dental Medicine.
